The adaptive immune system has the capacity to generate a large number of different antigen-specific receptors and hence can, in principle, provide defence against virtually any pathogen. However, there is a price to pay for this flexibility and sophistication, and that is that it takes several days to generate a protective response. In the meantime, the innate immune system must hold the infection at bay. The innate immune system requires receptors directed to structures which are commonly found in micro-organisms but which are not expressed in our own cells. One such structure is lipopolysaccharide (LPS), which is detected by our innate immune system because it possesses a high-affinity LPS receptor called CD14. This text underlines the importance of CD14 as a central component of the innate system. Issues examined include the structure-function relationships of CD14, the role of CD14 in cellular recognition of bacterial lipopolysaccharides, interactions of CD14 with components of gram-positive bacteria, soluble CD14-mediated cellular responses to LPS and non-inflammatory/anti-inflammatory CD14 responses.
